A combination of hard drugs and general ego problems. one of the major problems is that there were personality problems from the start, Axl's bad boy image against the Brian May like guitar genius of Slash, the quiet, un-rock like Izzy Stradlin in a heavy rock setting. The use of drink and drugs were also a factor, as the late nights, girls in their dozens and drink took their toll. Axl Rose seemed more intent on killing himself than his music, and one member was quoted as saying he did everything possible to kill himself.
They also lost several band members, such as the original drummer Steve Adler and eventually Slash himself. The arguments between Slash/Axl Rose centred around the drugs, and Axl's increasing apathy towards contributing musically.
As for a reunion, the members of both Velvet Revolver and AxlRose have denied everything, claiming it would not work and that times have moved on. Even if they did reunite, wouldn't it be possible to run Velvet Revolver as a separate but parallell project? Bernard Sumner of New Order did this with some success, combining his New Order commitments with Electronic, a band in which he collaborated with former Smiths guitarist Johnny Marr.

Axl's ego caused the break up. He's a great artist and all, when he actually comes through for his fans, he was constantly missing performances. Even now he's unreliable, for example Chinese Democracy and he's still canceling performances. It's never going to come out. I really hope that they don't get back together cause then Velvet Revolver would be done.

Slash was not happy with the treatment of the band and the fans and the public in general by Axel. Axel was a drunk who treated everyone bad would show up drunk and didnt respect anyone. He wouldnt come out and save the day when James Got burned over seas. He could have came out and saved the day but instead he decided not to play. This caused a riot.

I've heard musical direction varied among the members. Axl wanted to be bigger like the Beatles and Elton John while the rest wanted to stay true to their hard and heavy sound.

I assumed it was the fact that Axl's doughnut addiction meant that they couldn't afford to ship him round the country anymore - as for getting back together they are on tour this year!

I thought they were getting back together but I have just looked at their website and there is no news of that which is a shame. I really liked them. I'm not sure what the arguments were about.

too many drugs and too many large egos.
I doubt they will get back together as Slash just recently referred to Axel as a "wig wearing, botox bloated , over weight has been" or something along those lines
