One of the long term negative side effects of uncontrolled DM( Diabetes Melatis) is decreased blood circulation to small blood vessels (capillaries)through the body. The small capillaries of the eyes, feet, hands and genitals(esp ovaries and testes)are esp vulnerable. This decrease of blood flow( carrying water and nutrients to the area and carrying away waste products) can cause these vulnerable areas to be at risk for tissue death and sensation loss.
Many men (and women) with CONTROLLED DM have fairly normal sex drives and can "perform" and father/mother children as will as other males.

I have not heard of a decreased sex drive, but in can lead to impotence.
