it's not a law, rule or ordinance. most restaurants I've been to give water out as a courtesy though the water is usually warm and they may charge you for ice. Most gas stations will charge you for the cup and ice.

I do not believe so. It would be a waste of a precious resource if it were required, and an needlessly expensive water bill for the restaurant.
Most restaurants operate on a small profit margin. I'm happy to request it just when I need it. How about you?

No. At some restruants they have those soda machines where you press the button and soda come out and one of those buttons is water but you still have to buy a medium drink.

I'm not sure if its a federal law but in AZ you must allow people to drink water even out of your hose in the yard and all restaurants must give water at no charge if asked, but why not..... in AZ it can get up to 118 degrees or more in the summer ...its even hot at night too.

Not sure whether there is an actual law or not, however, how you get the water can determine whether or not you would be expected to pay...Tap vs. bottled. In places that the source of the water is questionable, it is safer to just go for the bottled...especially if traveling abroad!

NO.! It's just an optional service to "Offer" water routinely when you are seated in a restaurant and they don't NEED to "Offer" it. However, they can not REFUSE to give you water, should you ASK for it because that is required for safety reasons.
