As stated previously, she was killed once by the Master (in "Prophecy Girl") and later sacrificed herself in "The Gift". As for how many vampires she killed, according to "Dusted" (a very good Buffy episode guide), her total kill were 142 vampires, 74 demons and 2 humans.

Buffy was killed THREE times, the first by the Master in season one. (ok, technically four times, twice by the master if you count when she was killed during the episode where Cordelia wishes Buffy had never come to Sunnydale). The first death brings about the jamaican slayer, the second brings Faith, and the third..well, eventually the third brings about all the potential slayers. I can't for the life of me remember who kills her the second time, though the third time she dies for Dawn by diving into the portal that Glory, the goddess opens.
Not sure how many vamps she killed in all...I'm sure it's listed on some fans site, somewhere.

Buffy only died twice in all the seasons and the first time she was killed by the First evil vampire.
There is no exact count on how many vampires she has actually killed.

Only one vampire killed Buffy, The Master in Season One. She died at the end of season 5, but she sacrificed herself for Dawn.
