In my expert opinion (from watching Medium and Ghost Whisperer) sometimes it happens right away and sometimes the spirit has some unfinished business to attend to. For more details you should probably talk to Melinda or Allison.

only when the spirit excepts its time in this body is done if there is unfinished business it will not cross over.
