... some guys actually get off on that sort of stuff, you'd be surprised... he might be perfectly aware and even encouraging her roaming eye-- you just never know what goes on behind closed doors.
It's really none of your business to tell him, and I'm sure you know it.
It would probably ruin your friendship, if you really value that.
Obviously "everyone else" knows (people talk). It's not the 1800s anymore (so not like she's being ostracized, obviously). If everyone is being discrete about things... that's the way it should be!

You should tell her to tell him first and tell her if she doesn't then you will. He should know especially because if she's sleeping around she could pick something up from someone else and give it to him. And if he's a good friend of yours you don't want to lose him when he finds out you knew and didn't do anything. And if she's mad at you now then so what? She';s ruining her marriage and if she wants to get out of the marriage she should tell her husband instead of sleeping around. Or maybe there's a problem they can fix but they won't know until she comes clean. No pun intended.

Was this before or after they got married. If it was before, there's no need to mention it, if it was after then there's still no need to mention it as it's not your place to tell him unless you and he were both friends even before yous both met her.
