I don't think one should have to also give an excuse to leave work early, a simple "I have a personal issue to handle and need to leave eary" is perfectly suitable

I work at a college library and my excuse is having to study for a test. They are very laid back there and always let me go.

Have you complained about any menstrual cramps within the month? If not, use that. If your boss is a guy, he'll just be horrified and say "Sure go, go!"

If your boss is a woman then she'll totally understand and probably be pretty nice about it.

If you have a job where you have to ask, my advice would be to get another job. But I know that's not always that easy, so the sick thing usually works as long as you don't over use it.
