At one time, it used to be us here in Canada. However, with all the government cutting and slashing, our services have eroded to the point that we now in effect have a two tiered system. That is, most services are paid for by our tax dollars and don't cost a cent when you use them, but in some areas if you don't want to wait, you can go to a private pay clinic.

Probably Germany. Despite their outrageous taxes, every citizen cares for each other, unlike here in the U.S. where everyone is out for themselves.
