Do not speak of her birthday until that day. Pretend as if you have forgotten. And then,bang...give her a surprise visit on her birthday eve, with flowers and a special gift and if possible a hand made card with real touching wordings{You could even cut and paste yours and her pictures on it} to make it more creative and personal.
[another clue:choclates of different kind, all packed together in a beautiful box also make a good gift]
And I am sure, a girl that age is really going to feel on top of the world.

A scrapbook along with whatever present you're thinking of will probably be a good route to go down. Photos, stories other friends input. These things generally bring back good memories when sifting through stuff in the house later on in life. Everytime I see my signed yearbook from school, I end up sitting down an reading what everyone wrote. So you could do something similar.

Go online and get a star named after her. You can do this now quite cheaply and you get a special certificate saying where her star will be in the solar system.

I had an ex who loved animals but had some allergies, so i adopted her, through a charity, a whale. I know some zoos do this with their animals too, so you could take here out tell/show her which is hers.
