Mostly because it didn't have to when the network was first set up. .gov and .mil are restricted to US anyway, and before TLDs were a big deal, most US companies were using .com anyway. Most american users consider the lack of a TLD to indicate US anyway, so it's unlikely that there will be much of a push for new web sites to use an explicit US TLD.
