Not rinsing them off well enough before putting them in the dish washer. If there is food stuck to them it will just get splashed around and end up back on them.

A good rinse of the dirty dishes getting rid of the left over food particles before loading your dishwasher will prevent such things.

If you have hard water, scale and lime build up in the dishwasher, best way is to run a wash cycle with lime-away. then run a wash cycle with vinegar. After you do the wash cycles put in your dishwasher a bottle of jet-dry. Sometimes if you do not occasionally clean the dishwasher without dishes in it the areas where water is shot at your dishes gets plugged and you are not getting your dishes clean...try this remedy hope it works.
