At medical examination, the doctor will hold the scrotum in his hand ask the patient to cough. If both the testecles go up during coughing, he is declared fit. Otherwise, he is not.
But I don't know why only one testicle go up that too when breathing.
See a physician to rule out any complicaitons

A man's left testicle is attached to his right side lung and vice versa. So, as you breathe in and your lungs fill with air, the testicles go up and when you exhale, the testicles go down. Sometimes, the movement is not enough to notice, but in your case, on your left side, it is enough for you to notice.

Testicles move closer to, or away from, the body, according to the body's temperature. After a cold shower, they'll be as close to your penis as they can be. Lay out in the sun, and they're going to hang loose.

because your LEFT testicle is connected to your lungs. whereas your RIGHT testicle is connected to your EYELIDS.
to prove this, grab a pubic hair on your RIGHT testicle and yank it down hard. you will observe that your eyelids will close or pull down at the same time....
