Yeah as he was dying he imagined all the people around him as the parts that they played throughout the whole movie. The woman that played the waitress and he supposedly loved from afer was his fiance in real life, the blind man that he said was his father was his father in real life, and the wonam that was his mother was his mother in real life also and they were all killed in the car accident.

It was a cheap, total RIPOFF twist ending that made no sense. I understand that if you give the audience too many things to think about--they may actually THINK they UNDERSTAND something when there was NOTHING to UNDERSTAND. Did you notice the many shots when there were people dressed exactely the same--like they were all TWINS--walking around? It all comes down to STYLE over SUBSTANCE--and the filmakers were hoping that it would be easier to NOT think about any of it when it was over. Watch Jacob's Ladder instead.

Yes he was dying after the car crash. Then he died it ended. The characters in the movie where based off people who were around him while he was fading away.

The whole movie takes place in Ryan Goslings head while was dying.
