There are three answers to that question:

a)I don't know, but
b)If they have, we won't know until 50 years afterwards (top secret classifications and all that), and
c)Because we have to, and they alledgedly ARE needed for foreign intelligence. Although, I think the 'I' in CIA doesn't stand for "Intelligence". It prob'ly stands for "Idiot".

When they are doing a good job, we won't know (at least not for a long time after the success). That's part of the job. Unfortunately, when they screw up sometimes we don't know.

But it is a necessity that they do the job of gathering intelligence and acting on it. I know it's a scary scenario, but what can you do? If they put everything out in the open, that would compromise their ability to do the job.

I do not believe that any organisation which keeps all of its dealings secret from the people who pay for it can be totally above board.
