You have to contact the DMV office nearest you in PA, and file the necessary forms. If you qualify, then you pay your fee and the temp.permit will be mailed to you.

Depending on where in Penn you live.Your doctor must fill out either a doctors note stating your temporary disability and how long you will be out or there is a form hat you can usually pick up at you local police dept or village or town office to be filled out by your doctor and then returned.
