You need to file a petition for name change in court. It's not as simple as "filling out a form".

To change your birth certificate, you musy file a petition or complaint for a change of name in whatever court your state requires it to be filed.

You must do more than fill out a form. A judge must decide & approve. Don't forget to change your credit cards, mail, Social Security, income tax form, oh, and so many things!

go to ur local court and request a form to change ur name. u may need to pay a fee and tell the courts y u want yo change ur name but this should work.

you have to get them at a court of law or get you a laywer
