It's pretty much the same thing as a "Indian monkey spanker" or a "Florida dolphin waxer"

I believe it's a machine for making Arkansas Taffy. Since taffy requires a lot of kneeding and stretching, the manual process could really inflate the cost of taffy, thus J. Winston Gollows of Little Rock, AR invented the Arkansas Taffy Puller in 1903. Hope that helps.
