The death of the hero or main character is always a suprise. Like when Dumbledor dies in Harry Potter - til the very end of the book you're hoping someone or something will step in and save him.
Also - the bad guy winning - very unusual so always a shock and a suprise.
The main character finding out something totally shocking like the bad guy is actually their father (Star Wars Luke and Darth Vadar), or that they have been a ghost all along (ie. Sixth Sense).

I read a story one time that ended "John woke up safe in bed. It was only a dream." I set fire to the book and refuse to read anything from that author ever again. It's a cheap meaningless ending.
One my favorite surprise endings is a from a short story. The setting is a very dark room and no one could see the others clearly. The ending revealed that one of the main character had been dead through the whole story. Everyone thought she was angry and giving them the silent treatment.

Scaramouche by Rafael Sabatini has one of the greatest twists.You find out why the fate keeps throwing Andre-louis aka Scaramouche and the Marquis de la Tour on opposite sides wherever they go.Also almost all the O.Henry short stories have surprising twists at the end.Lost oppertunities,chance encounters all lead to the 'twist'.eg:the cop and the anthem,last leaf,gift of the magi
