Go to the Department of Labor.
They will give you the forms and the info.

It may differ in certain states but to my knowledge you have to have work on that job up to six months. Loose your job at no fault of your own. You go to the unemployment office in your city/state and take a # and have a seat. Once called they give you forms to fill out and ask you to wait to be called by an advisor. Once the advisor gets you they ask you questions about your job activities and why you were dismissed. Try it you'll like lol
