There is nothing technically WRONG with the sentence, although you could argue that twelve is redundant. If you said the break was at noon, it's clear when the break was, however, if you said the break was at 12, the hearer might wonder whether the speaker meant noon or midnight.

Its correct. Noon and 12 oclock is the same thing but its still right to say twelve noon.

I don't think you have to say twelve because there is only one noon, so maybe "The break was at noon" ?

that should be changed to my because you are talking about one thing and that is the break and use my and the sentence and it will be like this"my break was at twelve noon." and by the way that sentence is past tense so my is past tense.
