It's just a saying. To say something like, "he got out of there like a bat out of hell" just means he got out of a place really fast.

The Devil is said to rule in Hell. Hell is a place of eternal flame. For millenia people have watched volcanos erupt lava and molten rock (among other things), causing fires and general mayhem of that sort. So Hell, a place of eternal flame, must be under the earth, because people see glimpses of it when volcanos blow their stack.
Bats generally live in caves. Caves are in the ground. And if caves go into the earth, where Hell seems to be, then bats must reside in Hell with the Devil. Ergo, bats (when they come out to fly around and mess with people's superstitions) must be flying out of Hell.

The saying "like a bat out of hell" is because of the intense heat in hell. Wouldn't you run from a supremely hot and tortuous situation?
