The thing about the answer regarding the US Patent, it is true. He did create shoes to incorporate the 45* lean into his stage performances. As you said, this was patented in 1993, the Smooth Criminal video was shot in 1988.

That does not answer the 45* lean in the video. It has been speculated that Michael incorporated magnets or wires into creating the lean for the video.

The truth is that any answer would be speculation. That is a video/movie wonder that no one has been able to crack. My answer would be magnets because wires would be to long and thick and would be seen in the video from that particular angle.

The invention outlines how a special pair of shoes attaches to a stage to allow leaning forward beyond the centre of gravity of the wearer of the shoes.
