Active should be: I eat a peice of bread.
Passive is: A piece of bread is eaten (by me).
The 'by me' is in brackets becasue, generally speaking, the whole point of using the passive voice is because who or what did it is not important - or sometimes unknown.
This sentence sounds strange because in most imaginable circumstances, the 'I' would be the most important element - and the focus on the 'piece of bread' unimportant.
The passive voice is very useful when the agent is unknown. E.g. 'He's been robbed!' or 'Her car was stolen from outside her house!' etc.

"A piece of bread was eaten by me." It's actually better to use the active voice instead of passive since active is more clear to your reader. "I ate a piece of bread." See? Nice and clear. Passive sometimes confuses people, for some reason. If you're a budding writer or playwright, good luck with it and have fun! =)

The object in the other sentence, bread, becomes the subject. The verb becomes past tense, and you add the prepositional phrase "by me".
