Most of the answers above only address COVERING up or masking the odor. A good detail job at a reputable shop may do the trick... if not there are some awesome Air Purifiers available that would likely be able to actually neutralize and eliminate the odors permanetly. I have one called the Fresh Air To Go from Ecoquest in my '02 Durango. I smoke in this vehicle daily and my little dog rides along with me just about everywhere. The Ecoquest line of air purifiers are only available through authorized independent business owners.
The one I got mine from can be found here:

I have one for my Durango and it smells just like showroom NEW everyday I climb into it. I also have one of their terrific products in my home and at my bar where everybody smokes daily. There is nothing else like em that I've been able to find. Most dealers will even let you evaluate a product for a week for FREE...

First air it out. Open all the doorways for an hour or so and basically permit some frsh air bypass by. Then get it exact: a carpet shampoo, vacuum, sprint board cleansing, the works. The shampoo itself could desire to take most of the scent out. And something will make it scent alot greater beneficial. After that, spray some Oust or Febreeze for any left over smell. good success!!

Buy the cheapest can of coffee you can find. Open the container and set in in your car for a few days and the smell will diasppear. This worked on cat urine so I am sure it will work on smoke and dog. Good luck!

open and put a box of Bounce,Snuggle or some other fabric sheets for the dryer under the seat. It will be amazing. Make sure and use one you like the smell of because it will be all you smell. Only use one box or it will be overpowering.

Have you car detailed (inside), tell them that, that is the main reason you are having it detailed and ask them to preform an odor removing cleaning..... That should take care of it and leave your car smelling yummy (depending on what ever fragrance you decide to have it cleaned with).
