The one that comes out first, was born first, so it's older than the one that is born later...making the FIRST child the elder. Same as regular kids that aren't twins...

Time and age are based on years, months, days, hours, minutes and seconds. When comparing a person's age, the person who has the most of these things is considered the oldest. Since birth - and therefore age - is determined by the time of "exit," it's the two that is born first that is the oldest.
