Check out the above link - find your State (assuming you live in the US) and if your doctor is a Medical Doctor click on the appropriate link for your state - this should bring you to a site where you can verify if your doctor has a liscence, any sanctions against them and you can usually file a complaint against a doctor.

The AMA , also called the American medical association. You can also check his background online for a fee, to see if he has had many lawsuits, restrictions or if his licsense was pulled in another state. If he has lost hospital priveledges somewhere then he has problems, get away and see another doctor.

If it is a private doctor, then send your report to the board of medicine in your state.
If the doctor is part of a group (like a hospital or a clinic) then report him to the manager or president of the hospital. You can always report doctors to the board (but I think you might need a lawyer to do that, you will need to check).
Or if you don't want to do that, report him/her to the media...

If you feel you have a legitimate complaint, ie, nothing petty, as physicians train long and hard for their titles and pay malpractice insurance fees that are outrageous, you should contact the AMA (American Medical Association) in your area. I imagine they could point you in the right direction from there.

To the Chief of Staff at whatever hospital this particular doctor is working at and to the medical board in your state.
you may even want to file charges if it is that bad, too.
