specialist doctor or general medicine but i am happy

Not really, all types of M.D.s are in demand.

Some M.D.s are motivated to become a specialist at the area of medicine they happen to be the most interested in and their practice consists of patients refered to them because of a specific ailment, but other M.D.s prefer to take up the challenge of general medicine where they must sort through complaints of all sorts and decide which they can treat and which need to be seen by specialists.

It is really a personal choice by the individual doctor, their interests, and the willingness to undergo additional training to be certified.

Well, it's not really important, more of a requirement towards the end. You can study general practice, but that's like a specialty too.
Either way, a doctor is a doctor, you pick what you like to do.
