Back in the good old days when a player played for his nation he was presented with a comerative cap to mark the occasion. Not sure if they still get them but basically the number of caps a player has indicates the number of times they've played for their national team.

For example, if David Beckham has 27 caps that means he's played for England 27 times. Like Dr Gus says players used to get a commerative cap, but now it's just used as a figure of speech. Besides, 27 caps sounds better than 27 shirts...lol
