They get attention and exposure. These people are exhibitionists, and some of them are probably hoping to be "discovered" by the entertainment industry.

a lot of the people who go on real world want to be in the entertainment industry and they they think maybe someone will see them on the show and want them to act, model, or sing. If you look into this or last months maxim Fitz from this season is in the magazine, but not as someone form real world as a model for tanning lotion I think.
