They speak with body language. They actually have a large vocabulary. they are also somewhat telepathic.
You should read the Monty Robert's book, The Man Who Talks With Horses. It will really give you an understanding of horse language

Horses communicate with us in many ways. For instance, if a horse feels tense, it can mean that he is fearful of something. If you are riding and your horse lays his ears flat back against his head, you are probably doing something to annoy him, same thing with a constantly swishing tail. Or he may be trying to tell you that he is in pain somewhere. Horses tell us when they want to come in the barn by hanging around the back of the barn or pacing close to it. They tell us they are hungry by calling to us when we walk in the barn. They tell us they hear something that may frighten them by pointing both ears forward and raising their head looking in the direction of the object. They tell us they are bored when they chew things that they should not, or playing with their buckets or grain tubs in their stalls. They tell us the flies are bothering them when they stomp their feet a lot and swish their tail and toss their heads in the field or pasture. When you have owned a horse for a long time, you can even tell by looking in his eyes what he wants. These are just a few examples. The language of the horse is endless. We just need to watch and learn to speak the same language....and never stop learning.

How do you communicate to your horse? He "talks" to you the only way he knows how - in his own body language. Watch him interact with oher horses and that will help you understand his signals to you. Facial expressions too - and this will help you understand his reactions to you. Monty Roberts' book is definitely a MUST READ>

they use body language to communicate. for example, if a horse has his ears flat on his neck he's angry, and if his ears are up and forward he is alert.

p.s. the book by monty roberts is "The Man Who Listens To Horses", not talks, but it is a fabulous book and i highly reccomend it.
