Maybe just spend one whole week/day to try to do what you can. And this time don't decide with your mouth but with your heart and mind. Make a friend come over and help with your homework that he/she knows or might have already done.

Your probably don't have any initiative to study. Here's a good way to get you back on track to where you need to be:
First set a goal - a REALISTIC goal and write it down in bold at the top of a piece of paper. Then under that write 5 different ways to achieve that goal, whatever the subject or matter might be. Now write down directly under your bold goal the first baby step you have to take in order to achieve that goal. Highlight it when you actually do it. Do all 5 things toward your goal and reward yourself with a nice relaxing bubblebath or a fattening candybar you've been denying yourself. I always keep a picture of something/someone up to remind myself why I'm doing it. I hope this helped and good luck with your studies. Just remember: baby steps! And when you get tired, set a egg timer to 5 minutes and step away until the buzzer buzzes.

Do study. As it is a continuous process. You tired at this age then why don't you think about those who are still learning at the age of 80 and above. Be brave and do it. I am here with you. You'll will certainly finish it on time.

Well, prioritize and it's okay if you don't get everything done. Do what you can and everything else will come out smoothly. If there is something that happened in your family, I think that your teachers will be a little forgiving.
