There are actually 32 teams in the World Cup. Teams qualified by playing within their regions for the past three years. Europe, Asia, Africa, South America, North America, Oceania are the regions represented. At the beginning of the qualifying rounds, there were around 200 teams trying to get into the World Cup.

The number of teams from each region allowed into the World Cup varies by region. Regions with more skilled teams get more spots (this year, Europe has 13 teams represented, which is the most, by far).

This article shows how many teams from each region are chosen, along with many other details:

There are different leagues who play for the World Cup Finals. US plays the North, Central American, Caribbean League. Then there is the South American league, and a few Eurepean Leagues and African leagues (because there are too many team competing to have just one league. They all play round robin and the top teams go to the World Cup Finals.
Don't know the number of teams eliminated.
There are a very broad range of ability and skill among the teams and the leagues. Watch the game and you will see.
