how many times does a dogs teeth fall out. only once as puppies or do they continue to do so thru out life?

They should only fall out once during puppy-hood. If they fall out later in life, they are loosing teeth just like adult humans do- they won't grow back.

all puppies lose only 2 teeth and as rule they chew on there toys because there teeth are growing. Chewing helps the teeth, when 2 years
you will notice two teeth are missing, only from the upper jaw section.

Just like humans pups lose their teeth & they r replaced by permanent ones.
