any ideas on a marketing survey for a report?

i have a marketing report due and i need to create a survey to base it on.

Step 1 - Create a thesis. Determine what sorts of data you want to prove/disprove/or learn about the product/service in question.

Step 2 - Formulate a series of true/false questions based on your thesis.

Step 3 - Express these true/false questions in a manner which allows response along a 1-10 or 1-5 scale.

Step 4 - Include a few double checking questions. Ask the same question (with different wording) more than once.

Step 5 - Present the survey.

Step 6 - Tabulate your result.
