Its tough getting past the "gate keeper" but if you bring "offerings" you will come alot closer to getting to the doctor. You may have to go to the business office manager 1st. If you do a good enough job, they will probably take it to the doctor.

Skip the office visit, start hanging out at golf courses. Seriously, look for a local hospital-affiliated wellness/fitness center. Many doctors will be members there. Stay in shape and make business contacts.

You will probably have to deal with the staff that is part of the job. i am an office manager, and no body gets past me to see anyone with out the o.k. from the person they want to see..
you could always bring in donuts... or nice little trinkets..
