I've always heard it used as out of control and angry.
Example) He is getting belligerent again.

nothing, but BELLIGERENT means someone who fights, and as an adjective means 'warring' or 'wanting to fight'.

So, if someone has a belligerent tone of voice, it means they sound like they want a fight.
Also 'the belligerents of World War II' means the countries who were at war with each other.
