Depends on where the pain is. Often a small amount of a hormone gets released during your period called "Relaxin". its job is to make the ligaments looser in case you become pregnant. If some of the ligaments are already strained from stress placed on them, then the nerves that register pain may be activated. Cramping of the uterus as it tries to expel the blood may also triggers pain receptors in the spinal column if it gets too intense.

Cramps are normal - one of the best ways to deal with it is to start taking Ibuprofen(like Advil) regularly the day BEFORE you are supposed to get your period, and continue through however long your cramps usually last. Also, a heating pad (or now they have portable ones you can wear under your clothes) can be a Godsend.
If you are experiencing pain that is not helped by these measures, then you need to talk to your Dr. Also, if you are having other symptoms like pain at other times of the month (like when you ovulate) or pain with or after intercourse, diarrhea, etc. you need to talk to the Dr. It could be a sign of another problem like a cyst, endometriosis, or irritable bowel disease.

Many young women tend to have dysmenorrhoea (menstrual pain). Usually, common medication such as ibuprofen should help. However if the pain gets worse or longer, or if there is bleeding or you think you are pregnant then you need to seek medical attention. If you also have chills or fever please get medical help.
