It depends what you're trying to do. For loans (car, home, bank) or mortgages, there's a standard form that's similar to the one linked below, the 1005 (each bank will have their own version, but it has the same general info). This form will be sent to the employer(s) from the bank making the loan and will require the employer(s) to fax and/or mail back directly to the bank. For general employment background (like an employer verifying past employment for a candidate), usually the recruiter will just call and do it over the phone, or make up a short form and fax to the other companies. The application should have a clause releasing all parties of liability that the employee signs. Employers do not have to verify the wage if they don't want to.

Most businesses use an internal form. If it is not for/from the Government, it is a basic form. There are only so many things you can inquire into regarding someone's employment.
