The Painted Bird by Jerzy Kosinski.
It's a story of a gypsy boy struggling to survive on his own during WWII. It is the single most horrifying book I've ever come across. Its searing, poetically graphic descriptions of wartime atrocities are unmatched by any book or movie. I had to put it down several times because some scenes were too disturbing.

One of the most horrifying books I ever read was The Door to December by Dean Koontz in which this scientist experimented on his young daughter. He encased her in a vat for days at a time, even catheterizing her in order to keep her there for longer periods of time. Horrifying.

Any of the Books of Blood by Clive Barker. Very horrifying in a sick way.
If your talking about books that scare you, hmmm, probably Intensity by Dean Koontz.

Cry To Heaven- Anne Rice, This book was so disturbing, but really good. It revolves around the castrati opera singers, and particularly a 15 year old boy whose brother had him kidnapped and castrated for vengeance and sent him to live with the maestros. It is a great revenge story, but really heartbreaking and hard to read because it is so terrible what is done to these boys.

William Peter Blatty's "The Exorcist". The book is a whole lot scarier than the movie. Another one is "The Stand" because some of that could realistically happen

I have to say Blatty's "The Exorcist," too. The movie scares you, but the book goes in depth about why you should be scared. Seriously, I read that book in one day, with my back against the wall, and all the lights in the house turned on.

Flowers in the Attic by VC Andrews...I first read this when I was a teenager, and when I was done, I felt like I had to have a shower or something. It left me with very negative feelings. I could not read any of her other books.

I read about half of Possessing the secret of Joy by Alice Walker, but the descriptions of female circumcision were too much for me, so I had to stop.
(I hope that's the right book, the one where one of the characters meets Carl Jung)
