All insects have 5 eyes.

Many arachnids have 5 or more eyes including spiders and scorpions.

Many mussels such as clams have numerous eyes, often 20 or more.

Box jellyfish have 5-15 eyes.

The only chordates to have more than two eyes are the jawless fish: lampreys and hagfish. These fish have a third eye located in the middle of the head.

The tuatara, a reptile from New Zealand has a partially developed third eye in the middle of its head but it can't actually see with it.
