That's a nickname he got in his native country, Cuba. It was used to indicate that they respected him as a very good pitcher.

El Duque inherited the name from his father, a Cuban pitching legend also referred to as "el duque" (the Duke). Hernandez's father got the nickname because "of his penchant for dressing, and pitching, in winning fashion."
