It's one piece connected for back and seat, green stripes (but not important), I've looked on the web all day. It was originally bought at Lowe's around 5 years ago and all I can read on the tag at this point is sheen tex fabric and the company name that starts SHA (the rest is worn off).

Take your old cushion to a local upholstery shop. Select your fabric and have them recover your old cushion. I'll bet it will be cheaper, and I know it will be quicker, than trying to find the manufacturer and a replacement.

If you have a Farm & Fleet or Fleet Farm around you that would be a good place to look.
