It's happened twice: Harry Chiti was traded by the Mets to the Indians for a PTBNL in 1962, then became that player several months later.

The second time was in 1987, when Dickie Noles was traded by the Cubs to the Tigers, then the deal was reversed a month later.

I believe you are thinking of Rob Ducey (Phillies --> Blue Jays --> Phillies), but according to this ESPN article, it also happened to Harry Chiti (Indians --> Orioles --> Indians):
