I would have the fans and/or the radiator itself checked.

Chryslers are notorious for air in the cooling system, it probably needs to be bled , which is a pain, i had a Dodge Intrepid that the dealer had to work on 3 times in one day, every time i drove off the temp would peg in the red

Try getting the channels in the block checked out to make sure they're not clogged... I have a Dakota, and the channels that the coolant is supposed to flow through are kinda narrow... may be the same in the van.

If it has a clutch fan that could be your problem. Check the fan and see if it has any wabble in it. Whith the car off grab the fan blade and try to move it back and forth from engine to radiator. If it feels loose replace it. If that isnt the problem then you could possibly have a bad head gascet or a cracked head.

Some company, I am not sure which, makes auxillary radiators which can help immensely in hot weather or if your vehicle just runs hotter than hound on a fresh 'coon track.
