Most employers do this when issuing a standard uniform. I know the maintenance mechanics I hire have a standard $50 deduction from their first check for 5 uniforms. If the uniforms need to be replaced due to an employee mistake (rips, excessive staining, badge missing) then they get charged from their paychecks for the damages.
So to answer your question - yes the boss man can.

No, that isn't fair. First of all, you may not even want to wear the uniform, and your boss can't charge you for something you dont want. (nobody really can) Second of all, if you have to wear the uniform to work there, charging you for the uniform would be like charging you to work for him, and that just isn't right.

Yes, they can but you need to give them permission to make the deduction. You can also claim the expense on your taxes at the end of the year.

As far as I know, nothing can be deducted from your check (except for taxes) unless you sign a consent form for that particular item.

i have experienced both. some employers charge and some don't.
the ones who do, then that uniform is yours, even after termination.
the ones who don't, then that uniform is THEIRS and must be returned after termination.
