Yes, he can be killed. Cyclops' eye beams can cut through adamantium so he could cut Wolverine's head off if he had too. All they'd need is a small psychic command to tilt his head back. Cyclops could then lop Wolverine's head off and blast the rest of him to smithereens.
I don't think the internal source of Wolverine's healing factor is known (if I'm wrong let me know). He's put a claw through Sabretooth's brain and he turned out the same afterwards, so going through the eye won't do it. His internal organs are vulnerable and it may be possible to obliterate his flesh and organs but if the healing factor originate in his bones or bone marrow, given enough time he may even recuperate from that. Completely destroying every part of him from his head to toe, I think would do it.
If his healing factor works on a cellular level then this may produce millions of Wolverine's..... That must be how he makes an appearance in 20 comics per month!

Many have tried. None have permanently succeeded. The closest he's gotten to dead was when Sabretoth used the Mutant Power Cannon (powered at the time by Leech) to disable WOlverine's mutant healing factor. After Sabretooth gutted WOlverine,Wolverine was dead for 23 minutes before his mutant healing factor kicked in again.

Yes he can be killed. One of the ways is to teleport his head away from his body (by Night crawler or someone similar) should Wolverine ever turn totally bad

In one alternate future, Wolverine was killed by being dipped in acid until only his adamantium skeleton remained.

Yes he gets lung cancer and dies has cancer is mutated cells and his own X gene fixes nothing since mutated cell seem to give you super powers. In the real world it only kills you:(
