Use both Ad-Aware and Spybot: Search and Destroy to clean the machine thoroughly. Ad-Aware catches things that spybot doesn't get and vice versa. Also consider getting a copy of CCleaner and Spyware Blaster. All four programs are free and when used in conjuction efectively block and delete a lot of spyware from your PC.
Download the programs, install them, update them, and run them. Also, run them at least once every 2 weeks. Even if you have the software on your computer, the software is utterly useless unless you run them and update them regularly.

AdAware by LavaSoft is about the best anti spyware software I know of. A free version is available. If it can't get rid of everything there is always the option of reformatting your hard drive and reinstalling the operating system. If you have to do this I would recommend getting some anti spyware software on your computer before going out to the internet again.

Have you tried reformating your computer?
If you haven't and have a windows cd or linux or whatever you have.
reinstall everything. Be sure to back up everything on your external drive or a partition and just remove the windows installation.
That way you know for sure all the spyware are gone.

If your current spyware is not able to destroy it, it might at least identify it, and with the name of the spywere you can do a search online on how to delete it manually, I have been in that situation a couple of times and it has worked for me

Also do weekly cleanup on your machine:
Clean up cookies, offline internet files, history Under Tools, Internet options.
Run Disk Cleanup and Disk Defragment under Start, Programs, Accessories, System Tools.

have you tried spybot search and rescue
and you have to stay off the sites that give you spyware like the porn sites and the casino sites
