Yup. And going against what previous people have said here, it isn't just from a man waiting too long to withdraw. We have had an incident (with a normal sized condom, average size guy) where the condom came off in the middle of sex. And by then, it'd been pushed in pretty far and was difficult to get out. Eventually it did come out on its own. But this is a fairly rare occurance. Possibly caused by too much friction. Most of the time, however, simple things can be followed to ensure the condom leaves with the man:

1) Lubricant. If the girl becomes dry, or has been too dry, add a water-based lubricant to the outside of the condom while it's on the penis.

2) The guy _must_ withdraw soon after ejaculation, and must not continue sex afterwards because there's a high chance he will go flaccid.

3) The guy should hold onto the base of the condom while pulling out to make sure that it stays with him.

Yes it is possible for a guy to leave his condom inside a female especially by accident because it's never intentional if they are using a condom. They must be afraid to get the girl pregnant or just make sure nothing is being transmitted. It's normal and it happens more than you expect. It's basically natural for something like that to happen.

Yes this possible. The reason this happens is due to him not rolling the condom all the way down or wearing a condom that is too big.

Yes, of course. There's always that chance. If the condom doesn't fit correctly and with lubrication, it could slide off the penis and into the female's body. It's very possible. When you two disengage intercourse, if the condom is missing, you might want to check.

yes, that's why it is important to remove the condom while the penis is erect after ejaculation. Also the right size of condom to use during intercourse is necessary. Check your local drug store, they have sizes to fit all people. If a condom is accidentally left inside, it is best to seek medical attention, don't be embarrassed it happens to a lot of people.

It's possible to one to come off inside a female, but unless the guy totally forgot he was wearing one in the first place and didn't try to take it off, and didn't mention it to her....that ain't no accident!
