Here is what I do.
Apply weed and feed.
Do not let the yard dry out or the grass goes into a dormant stage. (brown)
This is probably the hardest thing to do.
Resist the temptation to cut it short.
Never cut it below 2 1/2 inches. 2 3/4 to 3" is best. A 3" cut leaves the lawn looking lush and thick.
Instead of using fertilizer I use weed and feed, you can also buy weed and feed with grass seed in it.
After you get the lawn looking pretty decent keep adding grass seed through-out the season.
The last cut of the year I cut it 2 1/2" as I will get mold in the spring if it is left to long, but this might not apply to everyone. It depends on how much sunlight it gets.

I've managed to make mine thicker and darker- have a long way to go though, since I'm new at this myself.
Use fertilizer every few weeks. Don't over fertilize though, or it can scorch the grass.
For patches, in the fall or spring when it's consistently under 80 degrees F, take out any dead grass, aerate the soil, sprinkle seed, fertilize and cover with some hay. I use the Scott's Contractor Mix, which comes up light green at first, but after awhile (and more fertilizing) gets darker. You can also use that Scott's Lawn Soil for patches instead of fertilizing. (Put the seed on top of the lawn soil.) Cover with hay.
Keep the seeds moist but don't drown 'em.

Depending on where you live there are lots of maintenance tips for lawn care. Never water mid day, either early morning or at dusk, or both. Fertilize of course, but never in the hottest months, early spring is best. If you need to replant seed, try to use Kentucky Blue Grass, or Bermuda seeds. Kentucky Blue Grass has always worked best for me. Eliminate traffic on your lawn, so that you don't have worn patches.
If all that fails, you can always buy sod, but it is kinda expensive. And if you do it yourself it is really hard work.
