notre dame-montana, theisman, lamonica
cal-joe kapp, craig morton, vince ferragamo
these were the only teams I cold get that had 3. There are quite a few with 2, such as ucla, washington st, miami,stanford

You didn't specify start in a Super Bowl at QB, so one answer would be Notre Dame: Joe Theisman, the great Joe Montana, and Paul Hornung, another legend who played QB in college but subsequently started in the first 2 Super Bowls at Running Back with the Packers.
